3.5 YEARS EXTRADURAL INTRA-EXTRAOSSAL CHRONIC ABSCESS RIGHT
OCCIPITO-PARIETAL REGION.
Anamnesis
The patient came to the clinic with
parents05-December-2002 telling that there is a
growing swelling at the right occipito=parietal
region. They noticed that 8 months ago and it is
increasing in size.
On examination: The patient neurologically free.
Up on inspection the mass is hard in consistency
and there is no reaction of skin over it. It
hard to inspect the bony boundaries, but it is
13x12x7 cm in diameter. The cardio-pulmonary
pulsation can be felt.
The patient was sent for MRI of the brain, which
showed an extradural mass in the right
occipito-parietal region expanding over the SSS.
It is multilobulated with tick capsule. It
perforated the bony skull with defect around 5x4
cm and growing subgalialy.
The patient is positioned so the the mass is in
the upper point of the field. Skin incision over
it and dissection around it was performed. The
mass was detached from the bony edges and
separation from the dura was achieved. At the
end of removing the remnants of thick capsule
from above the SSS got a small tear, which was
stitches. Routine closure of the wound.
Smooth postoperative recovery.
She was sent to the ward.
Follow Up
The patient came to the clinic 21-January-2003
with MRI done 23-December-2002 with clean wound.
The final histologic result was an abscess.
Comments
The abscess with these specifications
needs surgical removal.
The bone defect in this age and with
presence of abscess must be left open. The children heal the
bone well.
