The patient was operated by me for lumbar disc 1997. He came to the clinic 05-February-2022 complaining of neck pain for 1 year with right shoulder pain and numbness both hands for 8 months with instability of gait.

On examination, the patient now is not limping with no scoliotic stance. There is weak extension both hands 4/5 and right triceps muscle 4/5. There is analgesia of the little finger right hand. There is slight pain when turning the right upper limb posteriorly and there is palpable bony prominence under the right claviculo-acromial junction without pain. The patient did not noticed it before the examination. There are signs of bilateral CTS. There is no apparent myelopathic syndrome.

The patient was sent for investigations and MRI cervical spine showed huge extruded disc C5-6 with malacia of the spinal cord. The right shoulder showing the mass which is an enlargement of the coracoid bone with partial tear of the right supraspinatus tendon with no compression of the right brachial plexus and normal arterial and venous flow without compression. EMG demonstrating bilateral moderate CTS with no pathological changes of the brachial plexuses.

Using the C-arm. the C5-6 level was identified and discectomy of C5-6 was performed and the extrusion was removed, until the dura was seen at the entire bottom of the removed disc cavity. EOS OSImplant 7x13x15 mm dimensions was applied. Routine closure of the wound. Smooth postoperative recovery. The power of the upper limbs improved. He was sent to the ward.

The patient after dramatic recovery came today 12-February-2022 telling that electric shock down to both legs took place yesterday (L'hremmite sign). He was sent for MRI cervical spine and cervical X-rays, which showed no abnormalities. It was explained to the patient that this reaction could have place, after decompression and as rebound effect and was started in steroid therapy and Gabatrex.

Recurrence in cervical discectomy is zero.
