YEARS EXTRUDED DISC L4-5 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the Shmaisani hospital 13-September-2014
complaining of LBP with left sciatica for 3 years
with exacerbation last month with numbness of the left L5
territory. MRI lumbar spine done
06-September-2014 showing extruded disc L4-5
with left downward migration.
On examination; the patient is limping with
exaggerated scoliotic stance. SLRS was 40
degrees with pain left side. There is almost
drop left foot with weak planterflexion left
foot 3/5.
The use of MICRODISCXS failed because the
patient is very obese and the instrumentation
did not reach the intended target.
Left partial flavotomy L4-5. Foraminotomy left
L5 root. The downward extruded
disc was removed in several pieces. Left sided
intradiscal cleaning L4-5 disc space.
Routine closure of
the wound.
Smooth postoperative recovery. The power of
the left foot became better.
Comments
The patient still has an estimated
postoperative recurrence around 7%.
