Comments
There is still an estimated postoperative
recurrence around 7%, because the disc space is still not
shallow.
Here an example, that
bilateral cleaning helped not to miss a large
piece, which was retrieved from the left side.
Anamnesis
The patient came to the clinic
21-May-2002
complaining of LBP with right sciatica
for 40 days with positive cough sign. MRI performed 15-May-2002 showing
huge extruded disc L2-3 more to the right.
On examination: The patient is limping, using
crutches for 2 weeks with exaggerated scoliotic stance
with bilateral sciatica. SLRS was 45 degrees with
pain both sides. There is weak dorsi and
planterflexion right foot
-4/5 with hypalgesia both L5 and S1 territories
both feet.
Decompressive laminectomy L2-3 with foraminotomy
both L3 roots. The huge extruded disc was removed
lateral to the right axilla and right sided
cleaning L2-3 disc space was achieved. Then left
sided cleaning of the disc space was achieved. The patient was put in Reverse
Trendelenburg position with Valsalva maneuver
and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the foot improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 02-June-2002
with clean wound and sciatica free and normal
power both feet. There is still numbness right
lateral thigh.
The patient then came 28-July-2003 with new MRI
ruling out recurrence. He was advised to be treated
conservatively.
