HUGE RECURRENCE OF L5-S1 DISC WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient was operated by me
22-November-2013 for extruded disc L5-S1
with right downward migration. The patient then
came 10-August-2014 complaining of LBP with
right sciatica for 2 months with weak
dorsiflexion right foot -4/5 and hypalgesia all
the right limb. He was limping with exaggerated
scoliotic stance with SLRS 45 degrees in the
right. The right quadriceps muscle was weak 4/5.
MRI dorsal and lumbar spine were requested. The
patient then came 25-October-2014 with MRI of
the lumbar spine done the same day showing
recurrence of L5-S1 disc with right foraminal
occlusion.
Refreshment of the old incision with exposure of
the right upper corner of the L5 lamina.
Neurolysis of the right S1 root. The extruded
disc was removed and further cleaning of disc
space of L5-S1. Inomed ISIS was used to study
the bad looking S1 root. It could response only
to 7 and above mA DNS.
Routine closure of the
wound.
Smooth postoperative recovery.
The power of the right foot became normal.
Comments
The patient still has an estimated
postoperative recurrence around 7%, because the disc space
still not completely shallow.
The healthy root must response to 1-2mA
DNS. This badly affected root was responding only to 7 and
above mA DNS, which must indicate the severe damage of the
nerve. The complete postoperative recovery of the power of
the foot immediately after surgery is in contradiction to
these data.
