YEARS EXTRUDED DISC L4-5, L5-S1.
Anamnesis
The patient came
to the clinic 21-January-2002 complaining of LBP
with left sciatica for 1 month with history of
DVT left leg.
MRI of the lumbar
spine showing extruded disc L4-5 left side with
stenosis both L4-5 and L5-S1.
On examination: The patient
is limping with exaggerated scoliotic stance
with SLRS was 40 degrees with pain left side and
weak dorsi and planterflexion left foot.
Decompressive
laminectomy L5, upper border of the sacrum and
lower half of L4 with foraminotomy both L5 and
S1 both sides. The disc of L4-5 was removed from
the left side. The patient was put in Reverse
Trendelenburg position with Valsalva maneuver
and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the left foot improved.
She was sent to the ward.
Follow Up
The patient the
came to the clinic 31-January-2002 with
clean wound with improvement of the power of the
left foot , but had swollen right foot and
advised to be seen by vascular surgeon.
Comments
The patient still
have an estimated postoperative recurrence
around 7% because the disc space is still not
shallow.
