SPONDYLOLISTHESIS L4-5.
Anamnesis
The patient came to the clinic 11-May-2014
complaining of LBP with left sciatica for 5
years with headache, neck and left upper limb
pain for 8 years. The patient is hypertensive
for 5 years.
On examination; the patient is limping with
exaggerated scoliotic stance. SLRS was 40
degrees with pain in the left. Weak dorsiflexion
right foot 3/5 with hypalgesia right L5 and S1
roots territories.
The patient was sent for new investigations: MRI
brain done 11-May-2014 showed periventricular
hyperdensities, but MRA of the brain was normal.
Cervical MRI showed bulges C4-5, 5-6 and C6-7.
MRI lumbar spine showed II degree
spondylolisthesis with with dynamic studies
confirming it.
Skeletonization of L3,4,5 laminae with their
lateral masses. Foraminotomy right L5 root.
Transpedicular fixation of L3,4 and L5 levels
was obtained using the Vitatech system with 6
monoaxial screws 6.5x45 mm, 2 rods and cross
connector. About 6 mm traction was applied to
the right rod to correct the scoliotic
deformity.
Smooth postoperative recovery. The power of the
right foot became normal.
Comments
The patient has spondylolisthesis and
structural scoliosis. Both need correction.
