04-OCTOBER-2006   RAKAN MUSTAFA FALAH  19 YEARS  TRAUMATIC SUBLUXATION C4-5 WITH FRACTURE LEFT C4-5 FACET.

The patient  was exposed to RTA 2 days ago without LOC, after what he complained of neck pain and both shoulder pain. The patient was transferred from another hospital with MRI of the cervical spine and cervical X-ray demonstrating locked facet C4-5 with fracture of the left C4-5 facet.

Neurologically, the patient was intact and considering the instability of the fracture, even after proposed reduction, the patient was operated: discectomy of C4-5 was performed and reduction of the dislocation was performed, after what fixation with one level miniplate with 16 mm length screws of Stryker brand was applied. Check images confirmed proper alignment of the bony elements.

Comments:

1.  There were similar cases during the previous years, that they were reluctant for surgery and most of them required surgery later after 30-40 days of insult. Since the subluxation is associated with fracture of the facet, fixation must be performed after reduction to avoid repeat subluxation after reduction.
