15-JULY-2015 RAMZI GEORGE ISSA HUGE LEFT
CONVEXITAL SUBACUTE SUBDURAL HEMATOMA WITH RIGHT SIDED PLEGIA.
Anamnesis
The patient came to the clinic 13-July-2015 with
clinical history of falling down 31 days ago.
CT-scan done 17-June-2015 five days after the
injury showing minimal left sided SAH over the
left hemisphere. The last 4 days got severe
headache with weak right side of the body. MRI
of the brain done 08-July-2015 showing a very
huge subdural hematoma over the left convexity
pushing the brain stem and mid structures to the
right. The patient for unknown indications
underwent angiography which was normal except
for vascular shifts.
On examination, the patient is alert, but with
profound right sided hemiplegia with walking
with the aid of 2 persons.
Burr hole was created in the left frontal and
left posterior parietal areas. An external drain
was inserted from the burr hole in the posterior
parietal down to the most inferior dependent
point of the hematoma. Washing with saline was
continued for more than 1 hour until the coming
out fluid regained clear color. Routine closure
of the wounds.
Smooth postoperative recovery. The patient was
sent to the ICU for 36 hour observation.
Follow Up
The patient's right sided hemiplegia resolved 2
hours after surgery.
Comments
The patient is a young male, and chronic
subdural hematomas usually occur with elderly.
When looking to the other
hemisphere and subarachnoid spaces are wide,
which could trigger the hematoma formation,
since the left hemisphere logically must be
identical with the opposite side.
