The patient was operated elsewhere 17-February-2013 for PLD L4-5 for left sciatica, then came to me and was operated for huge extruded disc C5-6 04-July-2014. She was using crutches and stopped using it one year ago. The patient then came to the clinic 02-February-2016 telling that she improved, but still complaining of right sciatica with LBP with numbness left S1 territory.

On examination: the patient is not limping with no scoliotic stance. SLRS was 70 degrees with pain in the right. There is weak dorsiflexion right foot -4/5 and planterflexion same foot 4/5 and hypalgesia left S1 territory.

The patient was sent for MRI lumbar spine. It was preformed 20-February-2016 showing recurrent extruded disc L4-5 with right foraminal occlusion. Dynamic studies ruled out overmobility.

The old incision refreshed. The dissection was carried out over the scar. Right L5 foraminotomy. The recurrent extrusion was removed lateral to the axilla. Right sided intradiscal cleaning L4-5 disc space. Using MultiGen, a bipolar pulsed mode RF with 42 Celsius, 240 sec, 2 Hz and 20 msec duration to right L5 root was achieved using 2 bended catheters 10 mm exposed length. The previous ugly scar was removed. Routine closure of the wound.

Smooth postoperative recovery. The power of left foot became normal. The patient is sciatica free. She was sent to the ward.

The patient having an estimated postoperative recurrence around 0%, because the disc space completely collapsed.

This is the 41st case using the BPRF mode with MultiGen. This procedure regained routine acceptance  it became a usual part of the spine surgery. Click here for reference.
