27-JANUARY-2007 REEM ATTALLAH JMEAAN 38 YEARS EXTRUDED DISC L5-S1 WITH SEVERE
COMPRESSION OF THE RIGHT S1 ROOT.
Anamnesis
The The patient came to the clinic
22-January-2007 complaining of LBP with right
sciatica for 3 years. Exacerbation of the
sciatica the last 6 months.
MRI performed 11-November-2006 showed bulge L4-5
and extrusion of L5-S1 with right downward
migration.
On examination: the patient had scoliotic
stance, limping during walking with shooting
right sciatica. SLRS was 50 degrees in the right
and 55 degrees in the left. She had weak dorsi
and planterflexion right foot 4/5.
Foraminotomy of the right S1 root was performed
and partial flavotomy of L5-S1 was achieved.
There is no epidural fat in this area.
Despite the fact, that the extrusion was not so
big, but it was severely compressing the root.
This extrusion was removed lateral to the axilla
and minimal cleaning of the disc space was
performed, because the disc space height was
very narrow.
Smooth postoperative recovery and normalization
of the power.
Comments
The extrusion was minimal, but the root
compression was severe, which caused her agonizing pain and
neurological deficit.
