10-FEBRUARY-2015 RIYAD MAHMOUD AL-NAHAWI 42 YEARS
EXTRUDED DISC C7-D1 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 08-February-2015
complaining of neck pain for 1 month with left
shoulder and left upper limb pain and numbness V
finger left hand. MRI cervical spine done
17-January-2015 showing extruded disc C7-D1 with
left foraminal occlusion. The patient is a known
hypertensive for 13 years.
On examination, the patient is in agonizing pain
when turning the head to the left, upward,
downward and bending
the head to the left shoulder. There is weak
grip and extension left hand -3/5. The left triceps muscle 3/5. There
is hypalgesia ulnar distribution left hand.
The patient has very short neck with bulky
shoulders, using the C-arm, the level of C7-D1 was
identified with difficulty. Discectomy of C7-D1 was performed
until the dura was seen entirely more to the
left. Using cage 6 mm height and Atlantis
Medtronic plate 27x5 mm with 4 screws 4x17 mm, fusion
of C7-D1 was achieved. All stages of surgery were
guided with C-arm. Routine closure of the wound.
Smooth postoperative recovery.
The power of the left upper limb became normal
immediately after surgery.
Comments
The patient short neck and bulky
shoulders, made the procedure difficult with deep wound,
that necessitate to slightly widen the incision.
