HUGE EXTRUDED DISC L4-5 WITH BILATERAL DROP FEET.
Anamnesis
The patient came to the clinic
22-August-2002
complaining of LBP with bilateral sciatica for 9
years with exacerbation last 2 years. The
patient many times falling when walking. MRI done 21-August-2002 showing
huge extruded disc L4-5 central.
On examination: The patient in agonizing pain,
limping with exaggerated scoliotic stance.
SLRS
50 degrees both sides with pain. There is drop
both feet with severe weak planterflexion2/5.
There is hypalgesia both L5 and S1 both sides.
Bilateral L5 foraminotomy with removal of the
extrusion lateral to the left axilla. Left sided
intradiscal cleaning L4-5. Inspection the right
side showed that it is not necessary to violate
it. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.
Smooth postoperative recovery.
The power of the feet improved a little.
She was sent to the ward.
Follow Up
The patient came to the clinic 08-September-2002
with clean wound and further improvement of the power of the
feet
and sensation.
Comments
There is still an estimated postoperative
recurrence around 7%.
The drop feet is unknown duration in this
case. The longer duration, the less expectation to recover.
