YEARS DORSO-LUMBAR CANAL STENOSIS.
Anamnesis
The patient came to the clinic 19-August-2007
complaining of right hip pain and right sciatica
with numbness of right L5 root territory for 3
weeks. She is unable to walk for long distances
with intermittent claudication. The patient was
operated 1992 for LCS.
On examination: SLRS was 80 degrees in the right
with pain. with weak dorsiflexion right foot.
MRI performed 21-August-2007 showing severe LCS
extending from D10-11, down to L4-5. The patient
was sent to perform MRI dorsal spine to rule out
more rostral stenosis , which was negative.
Decompressive laminectomy D11 and partial of D10
and D12 was done and all compressive elements
were removed.
Laminectomy of L5 and L2 and partial of L1 was
done, and drilling of all compressive elements
were removed. Routine closure.
Smooth postoperative recovery and normalization
of the power of right foot.
Comments
The patient underwent surgery 14 years
ago for LCS L3-4. The condition escalated, that the stenosis
was total to include the lower dorsal spine.
The previously decompressed level at L3-4 was
so narrow, that decompression was needed also.
Preservation of the D12 and part of L1
laminae was attempted to preserve more or less acceptable
alignment of the spine.
