The patient came to the clinic 06-June-2002 in wheelchair  with inability to walk with spastic paraparesis more profound in the right with weak both lower limbs with elements of left Brown-Sequard syndrome below D7. MRI dorsal spine performed 04-February-1999 showed extruded disc D8-9. She was operated at that time without improvement. She was operated another time through trans-thoracic approach and fusion of D8-9 was done using pedicular screws lateral left side. No improvement  and check MRI in October-2001 showed the severe compression still there.

On examination: The patient in wheelchair with analgesia below the right D7 level with weak quadriceps both 4/5, weak dorsi and planterflexion right foot 3/5 and 4/5 left foot, abduction and adduction of the knees 2/5. SLRS was 0 degrees both sides,

Decompressive laminectomy D8, 9 with foraminotomy left Th8 root. This root was coagulated and bisected to obtain room for visual inspection of the bony compression, which was drilled of and the bed was smoothed so as to eliminate any sort of anterior compression. The patient was put in Reverse Trendelenburg position with Valsalva maneuver and hyperventilation. No CSF leak. Routine closure of the wound. Routine closure of the wound.

Smooth postoperative recovery. The neurological status still the same. She was sent to the ward.

The patient came to the clinic 25-August -2002 with clean wound and telling that the spasms decreased and the power of the left leg dramatically improved. The hypalgesia descend to D1 right side,

the patient then came several times with gradual improvement and the last visit was at 15-October-2003 with the hypalgesia restricted to the right L1-2-3, full power left leg and dorsi and planterflexion right foot 4/5, but the adduction of the right knee still 2/5.

The dorsal disc is rare, but needs special expertise to handle it.

The spinal cord is very sensitive to compression and decompression rarely yield a favorite result.
