The patient came to the clinic 18-August-2002 telling to remove the construct in the upper dorsal spine, because she want to marry. To my remembrance she came to me at Al-Bashir hospital fully paralyzed at the level of D2 due to tbc of D3 with severe compression. I performed removal off the all involved structures in tbc, and as I will remember to all my life, how the heart was pulsating down the field. I used at that time bone grafts harvested from other bones to fill the gap and used this H-frame aided with sublaminar wires. The hooks are under C5 and C7 up and D4 and D5 below. The patient was under treatment for tbc for 5 years and she slowly improved until she became neurologically free.

On examination: The patient is neurologically and not complaining. She just want to remove the construct before marriage and the scar is an ugly one due to tbc fistulas before and after the surgery.

The old incision refreshed and parts of the construct exposed and step wise removal of the sublaminar wires. The nuts were relaxed and the hooks release. The built-in cross connector was cut and the whole construct was removed.  The edges of old incision debrided and cosmetic closure of the wound was achieved.

Smooth postoperative recovery. She was sent to the ward.

The patient came to the clinic 02-Sptember-2002 with clean wound and neurologically free.

This case is unique in that the H-frame was removed. It was intended to be permanent, but the patient asked to remove it after full recovery and want to have cosmetic appearance of the ugly wound.

As seen in the video, it is very difficult to remove the H-frame aided with sublaminar wires. I performed over the years more than 40 operations using the H-frame. Now it is obsolete and other devices appeared in the market.
