63 YEARS PLANUM SPHENOIDALE MENINGIOMA.
Anamnesis
The
patient, an Iraqi citizen came to the clinic
12-October-2011 complaining of general weakness
and headache for 4 months, for what MRI of the
brain was performed 17-September-2011, which
showed planum sphenoidale meningioma occupying
the anterior fossa and reaching the falx
cerebri, more anterior from the right side and
more posterior from the left side, pushing the
anterior cerebral arteries posterior and up,
touching the optic nerves without considerable
compression.
On
examination: the patient is neurologically free
except for the headache.
Monofalp bifrontal
osteoplastic craniotomy was done with reflection
of the bone flap to the right ear. Subfrontal
approach with mobilization and preservation of
the olfactory tracts. The meningioma was totally
resected and the matrix at the planum
sphenoidale was vigorously coagulated.
Inspection of the optic nerves and supraclinoid
both ICAa and the sylvian cisterns and the
pituitary stalk. No remnants of the tumor. The
pericallosal arteries became lax after the tumor
was completely resected. Considering that the
frontal sinuses were large, it was necessary to
harvest a muscle from the right quadriceps
femoris to fill the frontal sinuses. Strict
heamostasis.
Routine
closure of the wound. Smooth postoperative
recovery.

Comments
The patient was neurologically free with
preservation of the olfaction. The olfactory
tracts were anatomically preserved.
The patient was discharges 21-October-2011 with
preserved olfactory function and she was
neurologically free.
