REVISION OF THE ILIAC SCREWS AND REDIRECTION.
Anamnesis
The patient was operated by me
08-December-2025 for progressing chordoma.
The patient neurological status improved, but
check CT-scan of the pelvis done the next day
with ORS Visual program showed unacceptable
direction of the iliac screws, for what he was
brought to the operating room to redirect the
iliac screws.
The wound was opened and
further dissection lateral to sacroiliac joint
was accomplished to visually follow the track of
the screw to be kept inside the bone.
Calculation of the direction of the iliac screws
according to ORS Visual software was to put the
screws 20 degrees more medial in the saggital
plane. The right iliac screw was 70x9.5 mm and
to the left 60x9.5 mm in dimensions. Visual
control denied any violation of the iliac bone.
Inspection of the tumor cavity was filled by
hematoma which was removed and the neural
structures were swollen. Routine closure of the wound.
Smooth postoperative recovery. He was sent to the ward.

Comments
The iliac screws were redirected more
lateral and violate the gluteal muscles. Redirection was
mandatory to achieve prompt fixation.
This error was caused by abnormal pelvic
cavity with the anaplasia of the left femoral head and
absence of the iliopsoas muscle in the left side with thin
bone in the left side.

its sensor gives feed back about the localization.

Check CT-scan showing unacceptable direction of the iliac screws,
which needs 20 degrees more anterior, for what revision screws was
planned the next day. Notice the left iliac bone is thin in the left
side and severely atrophied the left iliopsoas muscle.
Check CT-scan performed 11-December-2025 with ORS Visual software
showing the iliac screws inside the iliac bones.
Notice: Not all operative activities
can be recorded due to lack of time.
Notice: Head injuries and very urgent surgeries are also
escaped from the plan .
