SEVERE LCS L2-3, 3-4, 4-5 AND L5-S1.
Anamnesis
The patient came to the clinic 24-May-2002
complaining of LBP and bilateral sciatica
for 3 years with
numbness both legs and intermittent claudication
and inability to walk more than 50 meters. The
condition is deteriorating. MRI performed
recently showing severe lumbar canal stenosis
L2-3, 3-4, 4-5 and L5-S1.
On examination: The patient is limping with exaggerated scoliotic stance
walking bended anterior. SLRS was 70 degrees with
pain in both sides. There is weak dorsi and
planterflexion both feet
-4/5 with hypalgesia both L5 and S1 territories.
Decompressive laminectomy L3-4-5 with upper edge
the the sacrum and lower third of L2 with
bilateral
extended L3, 4, 5 and S1 roots
foraminotomy with inspection of the disci at all
levels. The patient was put in Reverse
Trendelenburg position with Valsalva maneuver
and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the feet improved.
She was sent to the ward.
Follow Up
The patient came to the clinic 10-June-2002
with clean wound and sciatica free and normal
power both feet.
Comments
The sooner the surgical decompression on
progressive lumbar canal stenosis, the better the outcome.
Skyra MRI with all clinical applications in the run since 28-Novemeber-2013.
