16-MAY-2007 ZEINAH MUHAMED ABDEL-AZZIZ 9 MONTHS SUBACUTE PROGRESSIVE
HYDROCEPHALUS DUE TO AQUADUCTAL STENOSIS.
Anamnesis
The patient's parents came to the clinic 15-May-2007
telling that the head of the child is rapidly
enlarging the last 2 months with the child
becoming inactive and unable to hold the head.
MRI of the brain done 14-May-2007 showing
massive hydrocephalus. The head circumference at
birth was 34 cm , 42 cm at 5 months age and now
48.5 cm.
On examination: the patient has spastic four
limbs, but the feeding, swallowing, micturition,
defecation and vision are intact.
Medium pressure PS Medical VPS was inserted from
the right side after what the fontanel became
relax.
Smooth postoperative recovery.
Follow Up
The patient's parents came to the clinic
25-May-2007 with clean wound and lax
fontanel.
The patient's parents came to the clinic
04-June-2007 after suffering broncho-pneumonia
with functioning shunt. The patient was admitted
to the hospital for massive antibiotic therapy.
The patient then came 16-August-2008 with new
MRI performed 04-June-2008 with no
hydrocephalus, convulsion free.
The patient then came 16-March-2010 with
convulsions of the left side with secondary
generalization. Tegretol was started.
The last visit was 04-September-2016, still in
Tegretol with scoliotic spine. The patient was
sent for investigations, but he disappeared.
Comments
It is better to perform shunting in early
stages, before irreversible damage to the brain take place.
Any infectious process with a shunted
patient must be dealt with caution.
Skyra MRI with all clinical applications in the run since 28-Novemeber-2013.
