19-JULY-2008 NAHLA MUSA ARSLAN 38 YEARS
EXTRUDED DISC L5-S1 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis:
The patient came to the
clinic 12-July-2008 complaining of LBP for
nine months
with right sciatica for one month with
numbness of the right foot at the S1 territory.
On examination: the patient
in agonizing pain with scoliotic stance
with SLRS 60 degrees in the right and 80 degrees
in the left with pain. She
had weak dorsi and planterflexion right foot
3/5.
The patient was sent for MRI
lumbar spine which was
performed 13-July-2008 showing PLD L5-S1
with right downward migration compressing the
right S1 root.
Right S1 root foraminotomy
with partial flavotomy of L5-S1 was performed.
The extruded disc was removed from under the
axilla in two pieces, after what the root became
lax. The disc space was cleaned lateral to the
axilla from the right.
Routine closure of the wound
and smooth postoperative recovery with
normalization of the power of right foot.
Comments
The patient was in agonizing
pain due to severe root compression.
The expected recurrence in
this case is around the average, because the
disc height still not shallow.
The intradiscal material was
looking healthy. This factor could reduce the
expectancy for recurrence.
