61 YEARS POSTPROTON BEAM DEHISCENCE OF THE WOUND AFTER REMOVING
THE CHORDOMA.
Anamnesis
The patient was operated by me
several times, the last one
10-December-2025 for chordoma sacral area
with complications related to proton beam
therapy. The patient then came 3 months ago with
clinical picture of pulmonary embolism which was
treated accordingly. The amount of discharge
from the dehiscent wound dramatically regressed,
but the wound still open and the rods are seen,
mandating closure of the wound with rotational
flap with pedicle.
On examination, the patient is feeling very weak
and data for pulmonary embolism are silent, but
the patient ESR is 110 mm/h. Septic workup was
done and the patient started in Vancomycin 1 gm
twice daily and Samixon 1 gm twice daily. The
patient was given medication and sent for
investigation. The patient is telling that for
several days got agonizing pain when touching
the right upper corner of the wound.
CT-scan done showing the screws in place and
pelvis X-ray also was normal for him.
Prone position. The dehiscent
wound defect explored and the screws were
stable. debridement of the wound until all areas
got bleeding. An epileptic skin with
subcutaneous tissues with partial muscle were
created and were pushed under created tunnel
from the left gluteal area and transposed to the
defective skin area to fulfill the cavity. Both
areas were stitched and it seems that the skin
became whitish is color. He was sent to the ward.
FOLLOW UP
Still early now.
Comments
The skin must be closed and the adipose
tissue must fill all the cavity to prevent long term
septicemia.
